Around 30% of perimenopausal women report new spots or acne, and it does not confine itself to the face. Chest, upper back and shoulders are common โ and body acne at 45 is both more surprising and more stubborn than it was at 16.
Why it happens now
The same driver as facial hormonal acne: as oestrogen declines the androgen-to-oestrogen ratio shifts, and androgens stimulate sebaceous glands. The chest, upper back and shoulders have a high density of those glands, which is why they are affected.
Two menopause-specific factors make it worse than the teenage version:

Is it actually acne?

What works for genuine body acne

- Benzoyl peroxide wash, 4 to 5%, as short contact: apply in the shower, leave a minute or two, rinse thoroughly. Far less irritating than leave-on, and it works. It bleaches fabric โ use white towels and bedding.

The marks matter more than the spots

- Folliculitis from shaving or waxing.

When to see a doctor
